ConFirm™ Premium Mail-in Biological Monitoring Service - 12 Tests

1. Remove the test strips from the pocket marked “test strips.” DO NOT remove test strips from the glassine envelope, as this will invalidate the test. DO NOT remove or process the control strip. 2. Place the test strips in the sterilizer per the manufacturer’s instructions (i.e., in the lowest tray near the door, or in the center of the chamber where the load is most densely packaged). Run a normal sterilization cycle. 3. Upon completion of the sterilization cycle, return the test strips to the pocket marked “test strips.” Seal the flap closed. 4. Complete all required information requested on the envelope. We cannot process envelopes that do not have identifying information (i.e. customer # or phone #), or are left blank. Optional information will be recorded if it is included. 5. Mail in the envelope. 6. Test results will be available via www.crosstexbms.com.
