TERMS AND CONDITIONS

1. Program Eligibility

• Rebate Percentage: A 30% rebate is provided on purchases of eligible HuFriedyGroup products.
• Rebate Basis: Rebates are calculated based on total invoices from authorized dealers.
• Program Minimum: The minimum total invoice amount required to qualify for the rebate is $5,000.
• Program Maximum: The maximum total invoice amount eligible for rebate is $40,000.
• Clinician and Practice Eligibility: Both the clinician and the practice must meet all specified program terms and conditions to qualify.
• Graduation Date: Clinician must be within five (5) years of their specialty graduation date at the time of submitting the rebate request.

3. Clinician Submission Requirements

To apply for the Post Residency Program rebate, the clinician must complete and submit the following documents to hfgresidencyrebate@hu-friedy.com:

• Post Residency Rebate Request Form: This form must be signed by the doctor.
• HuFriedyGroup 30% Rebate Certificate: The unique certificate received by email.
• Image of Dental Specialty Certificate or Diploma: Required to confirm eligibility based on graduation date.
• Dealer Invoices with HuFriedyGroup Purchases:
  • Obtain a distributor invoice listing only HuFriedyGroup items.
  • If a HuFriedyGroup-only invoice cannot be obtained, the Regional Account Manager (RAM) is required to highlight all HuFriedyGroup products and their total purchase amounts on the invoice.
  • The doctor's name and/or practice name must be clearly stated on all submitted invoices.
  • All pages of every invoice must be included.
  • Important: Sales Orders are not accepted. Rebates are based solely on invoiced items.
  • Include credit memos for any returned items.

4. Application Process

1. Online Initiation: The doctor begins the process online by visiting this page.
2. Form Completion & Certificate Receipt: The doctor completes the Post Residency Rebate Form online. Upon submission, they will receive two attachments via email:
   • A unique HuFriedyGroup Certificate with a personalized certificate number.
   • The completed Post Residency Rebate Form, which needs to be signed upon final submission.
3. Eligibility Confirmation: To confirm eligibility (clinician must be within 5 years of their specialty graduation date), the doctor must submit a copy of their Dental Specialty Certificate or Diploma.
4. Invoice Submission: The doctor provides copies of dealer invoices for which the rebate is requested. These invoices must clearly show the doctor's name and/or practice name. Rebate checks will be payable to the person (doctor) or entity (practice or group) listed on the invoice(s). Invoices may be combined if dated within 4 months. Preferred shipping method is HuFriedyGroup drop shipment.
5. Submission to RAM: The doctor provides the RAM with all required forms and documents.

Practice Requirements: The enrolled Program Participant must be starting a new practice or joining a multi-doctor practice. Corporate DSO and group practice orders subject to authorization based on evaluation of existing agreements.
